Buy a commemorative birth certificate

Use this application form to buy a commemorative birth certificate for a birth or adoption that took place in Queensland.

To submit your application form:

  1. Answer the questions below—it should take you less than 5 minutes.
  2. Pay $28.25 by MasterCard or Visa.

Size

All Queensland commemorative birth certificates fit into a standard A4 picture frame (certificate size: 210mm x 297mm).

The Department of Justice is collecting your personal information to process your application for birth certificates under the Births, Deaths and Marriages Registration Act 2023. The information on this form may be provided to law enforcement agencies and to government and non-government agencies to verify the data. Access to this information or a certificate may be granted to any person who has adequate reason to obtain it, or who meets the requirements of the access policy.

Public holiday office closure

The Registry of Births, Deaths and Marriages on Level 32, 180 Ann Street, Brisbane will be closed Monday 27 January for the Australia Day public holiday. We will re-open on Tuesday 28 January at 8.30am.
